To help remove the guesswork, I’ve rounded up some of the best business plan templates you can access right now.
These are listed in no particular order, and each has its benefits and drawbacks.
What to look for in a business plan template
Not all business plan templates are created equal. As you weigh your options and decide which template(s) you’ll use, be sure to review them with the following criteria in mind:
- Easy to edit: A template should save you time. That won’t be the case if you have to fuss around figuring out how to edit the document, or even worse, it doesn’t allow you to edit at all.
- Contains the right sections: A good template should cover all essential sections of a business plan , including the executive summary, product/service description, market/competitive analysis, marketing and sales plan, operations, milestones, and financial projections.
- Provides guidance: You should be able to trust that the information in a template is accurate. That means the organization or person who created the template is highly credible, known for producing useful resources, and ideally has some entrepreneurial experience.
- Software compatibility: Lastly, you want any template to be compatible with the software platforms you use. More than likely, this means it’s available in Microsoft Word, Google Docs, or PDF format at a minimum.

While not incredibly in-depth, it’s enough to help you understand how traditional and lean plans are structured and what information needs to be covered. The templates themselves are more like examples, providing you with a finished product to reference as you write your plan.
The key benefit of using these templates is that they were created by the SBA. While they may provide less guidance, you can be assured that the information and structure meet their expectations.

3. SCORE — Planning workbook

You will likely have to remove the exercises in this template to make it investor-ready. But it can be worth it if you’re struggling to get past a blank page and want a more interactive planning method.

4. PandaDoc — A template with fillable forms

These templates are designed for sharing. They include fillable fields and sections for non-disclosure agreements, which may be necessary when sending a plan to investors.
But the real benefit is their compatibility with PandaDoc’s platform. Yes, they are free, but if you’re a PandaDoc subscriber, you’ll have far more customization options.
Out of all their templates, the standard business plan template is the most in-depth. The rest, while still useful, go a bit lighter on guidance in favor of tailoring the plan to a specific industry.

5. Canva — Pitch with your plan

Canva is a great option for building a visually stunning business plan that can be used as a pitch tool. It offers a diverse array of templates built by their in-house team and the larger creative community, meaning the number of options constantly grows.
You will need to verify that the information in the template you choose matches the standard structure of a traditional business plan.
You should do this with any template, but it’s especially important with any tool that accepts community submissions. While they are likely reviewed and approved, there may still be errors.
Remember, you can only edit these templates within Canva. Luckily, you only need a free subscription, and you may just miss out on some of the visual assets being used.
To get the most value, it may be best to create a more traditional planning document and transfer that information into Canva.

6. ClickUp — The collaborative template

Out of all the project management tools that offer free business plan templates, ClickUp’s is the most approachable.
Rather than throwing you into all the features and expecting you to figure it out—ClickUp provides a thorough startup guide with resource links, images, and videos explaining how to write a plan using the tool.
There’s also a completed sample plan (structured like an expanded one-page plan) for you to reference and see how the more traditional document can connect to the product management features. You can set goals, target dates, leave comments, and even assign tasks to someone else on your team.
These features are limited to the ClickUp platform and will not be useful for everyone. They will likely get in the way of writing a plan you can easily share with lenders or investors.
But this is a great option if you’re looking for a template that makes internal collaboration more fluid and keeps all your information in one place.

7. Smartsheet — A wide variety of templates

I’m including Smartsheet’s library of templates on this list because of the sheer number of options they provide.
They have a simple business plan template, a one-page plan, a fill-in-the-blank template, a plan outline, a plan grading rubric, and even an Excel-built project plan. All are perfectly usable and vary in visual style, depth of instructions, and the available format.
Honestly, the only drawback (which is also the core benefit) is that the amount of templates can be overwhelming. If you’re already uncertain which plan option is right for you, the lengthy list they provide may not provide much clarity.
At the same time, it can be a great resource if you want a one-stop shop to view multiple plan types.

8. ReferralRock affiliate marketing business plan

It’s not your standard business plan template. The plan is tailored with specific sections and guidance around launching an affiliate marketing business.
Most of the template is dedicated to defining how to choose affiliates, set commissions, create legal agreements, and track performance.
So, if you plan on starting an affiliate marketing business or program, this template will provide more specific guidance. Just know that you will likely need to reference additional resources when writing the non-industry sections of your plan.

Does it matter what business plan template you use?
The short answer is no. As long as the structure is correct, it saves you time, and it helps you write your business plan , then any template will work.
What it ultimately comes down to, is what sort of value you hope to get from the template.
- Do you need more guidance?
- A simple way to structure your plan?
- An option that works with a specific tool?
- A way to make your plan more visually interesting?

Whether you’re a long-time business owner or starting to think about launching a business , to-do lists pile up fast, and determining how to write a business plan—much less following a business plan template—often feels overwhelming.
But nearly 70% of business owners who have been there and done that recommend writing a business plan before starting a business. When done right, business plans have enormous payoffs. In fact, a wealth of data now exists on the difference a written business plan makes, especially for small or growing companies.
First, know your audience
Start with a clear picture of the audience your plan will address. Is it a room full of angel investors? Your local bank’s venture funding department? Or is it you, your leaders, and your employees?
Defining your audience helps you determine the language you’ll need to propose your ideas as well as the depth to which you need to go to help readers conduct due diligence.
Once you’ve got your audience in mind, you can start your business plan, which should include:
- Executive summary
- Company description
- Market research and growth potential
- Competitive analysis
- Product or service description
- Marketing/sales strategy
- Business financials
- Organization and management
- Funding request

1. Executive summary
Even though it appears first in the official plan, write this section last so you can condense essential ideas from the other nine sections. For now, leave it as a placeholder.
What is an executive summary?
The executive summary lays out all the vital information about your business within a relatively short space. An executive summary is typically one page or less. It’s a high-level overview which summarizes the other sections of your plan.
How do I write an executive summary?
This executive summary focuses on what’s often called the value proposition or unique selling point: an extended motto aimed at stakeholders.
You can follow a straightforward problem/solution format or this fill-in-the-blanks framework:
- For [ target customers ]
- Who are dissatisfied with [ current solutions ]
- Our [ offering ] solves [ key customer problems ]
- Unlike [ competing product ], we have [ differentiating key features ]
This framework isn’t meant to be rigid, but serves as a jumping-off point.

2. Company description
Your company description should contain three elements:
- Mission statement
These elements give context to the bigger picture, letting investors know the company’s purpose so your goals make sense as well.
What is a mission statement?
A mission statement is your company’s reason for existing. It’s more than what you do or what you sell, it’s about why exactly you do what you do. Effective mission statements should be:
- Inspirational to make others believe in your vision
- Emotional to captivate readers and grab their interest
Throughout each part of your plan, less is more. Nowhere is that truer than your mission statement. Think about what motivates you, what experiences led you to start the business, the problems you solve, the wider social issues you care about, etc.
How do you describe a company’s history?
Don’t worry about making your company history a dense narrative. Instead, write it like you would a profile:
- Founding date
- Major milestones
- Location(s)
- Number of employees
- Executive leadership roles
- Flagship products or services
Then, translate that list into a few short paragraphs.
Why do business objectives matter?
Business objectives give you clear goals to focus on. These goals must be SMART:
They must also be tied to key results. When your objectives aren’t clearly defined, it’s hard for your team to work toward a common purpose. What’s worse, fuzzy goals don’t inspire confidence from investors and other stakeholders.

3. Market research & potential
Outline your ideal potential customer as well as the actual and potential size of your market. Target markets identify demographic information like:
By getting specific, you’ll illustrate expertise and generate confidence. If your target market is too broad, it can be a red flag for investors.
The same is true with your market analysis when you estimate its size and monetary value. In addition to big numbers that encompass the total market, drill down into your business’s addressable market—meaning, local numbers or numbers that apply the grand total to your specific segments. You may even map your customer’s journey to get a better understanding of their needs and preferences.

4. Competitive analysis
Competitive research begins with identifying companies that currently occupy the market you’re looking to enter. The idea of carving out time to learn about each potential competitor you have may sound overwhelming, but it will be an extremely useful exercise.
Answer these questions about your most significant competitors:
- Where do they advertise?
- What kind of press coverage do they get?
- How good is their customer service?
- What are their sales and pricing strategies?
- How do they rank on third-party rating/review platforms?
Spend some time thinking about what sets you apart. If your idea is truly original, be prepared to explain the customer pain points you see your business solving. If your business doesn’t have any direct competition, research other companies that provide a similar type of product or service.
Next, create a table or spreadsheet listing your competitors (competitor analysis table) to include in your plan.

Describe the benefits, production process, and life-cycle of your offering, and how it’s better than your competitors.
When describing benefits, focus on:
- Unique features
- Translating features into benefits
- Emotional and practical payoffs to your customers
- Intellectual property rights or any patents that protect differentiation
For the production process, answer how you:
- Create existing and new products or services
- Source raw materials or components
- Assemble them through manufacturing
- Maintain quality control and quality assurance
- Receive and deliver them (supply chain logistics)
- Manage your daily operations, like bookkeeping and inventory
Within the product life cycle portion, map elements like:
- Time between purchases
- Up-sells, cross-sells, and down-sells
- Future plans for research and development

6. Marketing and sales strategy
Your marketing plan can be the difference between selling so much that growth explodes or getting no business at all. Growth strategies are a critical part of your business plan.
You should briefly reiterate topics such as your:
- Value proposition
- Ideal target markets
- Existing customer segments
Then, add your:
- Launch plan to attract new business
- Growth tactics for established businesses to expand
- Retention strategies like customer loyalty or referral programs
- Advertising and promotion channels such as search engines, social media, print, television, YouTube, and word of mouth
You can also use this section of your business plan to reinforce your strengths and what differentiates you from the competition. Be sure to show what you’ve already done, what you plan to do given your existing resources, and what results you expect from your efforts.

7. Business financials
If you’re just starting, your business may not yet have financial data , financial statements, or comprehensive reporting. However, you’ll still need to prepare a budget and a financial plan.
If your company has been around for a while and you’re seeking investors, be sure to include:
- Income statements
- Profit and loss statements
- Cash flow statements
- Balance sheets
Other figures that can be included are:
- How much of your revenue you retain as your net income
- Your ratio of liquidity to debt repayment ability
- How often you collect on your invoices
Ideally, you should provide at least three years’ worth of reporting. Make sure your figures are accurate and don’t provide any profit or loss projections before carefully going over your past statements for justification.
Avoid underestimating costs
Costs, profit margins, and sale prices are closely linked, and many business owners set sale prices without accounting for all costs. New business owners are particularly at risk for this mistake. The cost of your product or service must include all of your costs, including overhead. If it doesn’t, you can’t determine a sale price to generate the profit level you desire.
Underestimating costs can catch you off guard and eat away at your business over time.

Once complete, create a big picture representation to include here as well as in your objectives in step two.
8. Your organization & management
Your business is only as good as the team that runs it. Identify your team members and explain why they can either turn your business idea into a reality or continue to grow it. Highlight expertise and qualifications throughout —this section of your business plan should show off your management team superstars.
You should also note:
- Roles you still need to hire to grow your company
- The cost of hiring experts to assist operations
To make informed business decisions, you may need to budget for a bookkeeper , a CPA, and an attorney. CPAs can help you review your monthly accounting transactions and prepare your annual tax return. An attorney can help with client agreements, investor contracts (like shareholder agreements), and with any legal disputes that may arise.
Ask your business contacts for referrals (and their fees), and be sure to include those costs in your business plan.
9. Funding request
When outlining how much money your small business needs, try to be as realistic as possible. You can provide a range of numbers if you don’t want to pinpoint an exact number. However, be sure to include a best-case scenario and a worst-case scenario.
Since a new business doesn’t have a track record of generating profits, it’s likely that you’ll sell equity to raise capital in the early years of operation. Equity means ownership—when you sell equity to raise capital, you are selling a portion of your company.
Most small business equity sales are private transactions. The investor may also expect to be paid a dividend, which is a share of company profits, and they’ll want to know how they can sell their ownership interest. Additionally, you can raise capital by borrowing money, but you’ll have to repay creditors both the principal amount borrowed and the interest on the debt.
If you look at the capital structure of any large company, you’ll see that most firms issue both equity and debt. When drafting your business plan, decide if you’re willing to accept the trade-off of giving up total control and profits before you sell equity in your business.
The founder can access cash by contributing their own money into the business by securing a line of credit (LOC) at a bank or applying for QuickBooks Capital . If you raise cash through a LOC or some other type of loan, it needs to be paid off ASAP to reduce the interest cost on debt.
10. Appendix
Finally, assemble a well-organized appendix for anything and everything readers will need to supplement the information in your plan. Consider any info that:
- Helps investors conduct due diligence
- Gives context and easy access to you or your employees
Useful details to cover in an appendix include:
- Deeds, local permits, and legal documents
- Certifications that bolster your credibility
- Business registries and professional licenses pertaining to your legal structure or type of business
- Patents and intellectual properties
- Industry associations and memberships
- State and federal identification numbers or codes
- Key customer contracts and purchase orders
Your appendix should be a living section of the business plan, whether the plan is a document for internal reference only or an external call for investors.
Make your business plan stand out
Investors have little patience for poorly written documents. You want your business plan to be as attractive and readable as possible.
- Keep it brief. A typical business plan can range from 10 to 20 pages. As long as you adequately cover the essentials, less is more.
- Make it easy to read. Divide your document into distinct sections, so that investors can quickly flip between key pieces of information.
- Know your margins. List every cost your business incurs, and make sure that you’re assigning those costs to each product or service that you sell.
- Proofread. Double-check for typos and grammatical errors. Then, triple-check. Otherwise, you might risk your credibility.
- Invest in quality design and printing. Proper layout, branding, and decent printing or bookbinding give your business plan a professional feel.
- Be prepared in advance. Have everything ready to go at least two weeks ahead so you have time to make revisions in case of a last-minute change.
Updating your business plan
It’s a good idea to periodically revisit your business plan, especially if you are looking to expand. Conducting new research and updating your plan could also provide answers when you hit difficult questions.
Mid-year is a good time to refocus and revise your original plans because it gives you the opportunity to refocus any goals for the second half of the year. Below are three ways to update your plan.
1. Refocus your productivity
When you wrote your original business plan, you likely identified your specific business and personal goals. Take some time now to assess if you’ve hit your targets.
If you only want to work a set number of hours per week, you must identify the products and services that deliver the returns you need to make that a reality. Doing so helps you refocus your productivity on the most lucrative profit streams.
Also, use what you’ve achieved and the hard lessons you’ve learned to help you re-evaluate what is and isn’t working.
2. Realign with your goals
Do a gut check to determine whether all of your hard work is still aligned with your original goals and your mission statement. Ask yourself these questions:
- Are my goals still relevant?
- Am I still focused on the big picture?
- Where do I want to be a year from now?
- Will my existing plan still take me where I want to go?
These questions may be tough to answer at first glance, but they reveal your ties to your goals and what most likely needs to change to achieve new wins.
3. Repurpose your offerings
If your time has become more focused on small projects rather than tangible growth and building a valuable client list, consider packaging your existing products or services differently. Can you bundle a few things together?
You must deliberately manage your revenue streams, and that might require shuffling things around a little to focus on what is working for you.

An office space lease agreement is a legal document between a landlord and a tenant outlining the rental of office space, suites, or shared workspaces for office-related use. Rent is typically calculated on a price per square foot ($/SF) and is usually paid monthly, covering amenities, services, and utilities. The agreement details the obligations of both parties.
This is only for commercial use. It is prohibited to use for retail or residential purposes.
Renting Office Space In 8 Steps

Commercial Lease Application Form – Use this form to verify the tenant’s credit history and background.
Step 1 – Evaluate Your Needs
For Owners or Current Tenants:
- Assess the Space : Determine the exact square footage, amenities, and expenses covered in the lease.
- Subleasing : Ensure subleasing is permitted within the current contract if applicable.
- Rent Calculation : Calculate rental rates on a per-square-foot basis.
- Utility Allocation : Decide which utilities to include and which will be the tenant’s responsibility.
- Lease Details : Finalize lease terms to streamline the tenant search process.
For Prospective Tenants:
- Budget : Establish your rental payment budget.
- Space Requirements : Determine the desired square footage.
- Lease Duration : Decide on the preferred lease length.
- Office Class : Choose the class of office space (A, B, or C).
- Desired Features : Identify sought-after characteristics and location.
- Consider Stakeholders : Incorporate the needs of employees and clients in your decision-making process for a more effective workspace.
Step 2 – Search for Office Space
Start by exploring online resources such as CommercialSearch , LoopNet , Digsy , Crexi , and OfficeList to find available office spaces for general listings.
These platforms offer flexible lease terms, often allowing cancellation with just 30 days’ notice. Committing to a 6-month or 1-year lease usually results in better rates.
Step 3 – Arrange Property Viewings
Once you’ve identified potential properties, arrange viewings. During these visits, the landlord or property manager will provide details such as move-in dates, monthly rent, available space, and other specifics.
Step 4 – Lease Terms Negotiation
After selecting the best space, negotiate the lease terms with the landlord. Consider comparing two properties to secure the best rate. Lease terms can vary, and there are four main types:
- Triple Net (NNN) Lease: Tenant covers real estate taxes, property insurance, CAMs, and their utilities and services; landlord ensures the roof and parking lot are in good condition.
- Co-Working Lease Agreement : Landlord covers all expenses and utilities.
- Modified Gross Lease: Costs are shared as negotiated.
- Gross Lease: Landlord covers real estate taxes, property insurance, and common area maintenance (CAMs); tenant covers their utilities and services.
Step 5 – Tenant Verification
After a verbal agreement, the landlord will verify the tenant’s identity and financial stability using a Rental Application . This process may include checking financial statements and business records, as well as performing credit and background checks.
Tenants with low credit scores often need a lease guarantor to secure their rental agreement. This involves having another individual guarantee the lease by completing and attaching a lease guarantor form.
Step 6 – Make Sure You Have All the Required Documents
Below is a detailed list of supplementary documents that may be required:
- Property Registration Document : Provides proof of ownership or authorization of the landlord to lease the office space.
- Identity Proof: For both parties.
- Address Proof : Utility bills or bank statements for both parties.
- Stamp Paper : A stamp paper of appropriate value as per state regulations on which the agreement is printed or executed.
- Security Deposit Receipt : Sometimes, a receipt for the security deposit paid by the tenant may also be required.
- Witness Statements : Statements from witnesses who can attest to the signing of the agreement by both parties, if required.
Step 7 – Finalizing the Office Lease
Once everything is in order, it’s time to sign the lease. There are three ways to do this:
- Handwritten Signature
- Notary Acknowledgment : Used especially for large-scale leases done in the presence of a notary public.
Step 8 – Moving In
After signing the lease, the tenant will need to pay the security deposit (if applicable) and the first month’s rent. Once these payments are made, the space is ready for occupancy!
Specify the start and end dates of the lease. Typically, leases for smaller spaces (under 2,000 SF) run for 1-2 years, while larger spaces often require 3-5 year commitments.
Tenants may request renewal options at predetermined rent increases, often tied to the Consumer Price Index (CPI) .
Rent usually includes a base amount paid monthly and a share of property expenses such as real estate taxes, property insurance, and common area maintenance (CAM) costs. A security deposit, typically one month’s rent, is also common to cover potential lease breaches.
Specific Use Clause
This clause restricts the use of the space to office-related activities, ensuring quiet enjoyment for other tenants. Common restrictions include:
- No food preparation.
- No medical procedures.
- Limit noise levels (e.g., no activities above 80 decibels).
- No retail operations.
- No hazardous materials.
Delivery of Space
Landlords can deliver the space in three ways:
- As-Is : Tenant accepts the space in its current condition.
- Vanilla Box : Space is delivered with basic finishes like white paint, ready for move-in.
- Custom Build-Out : Landlord completes the space according to the tenant’s specifications.
Landlords often include amenities such as high-speed internet, dedicated parking, 24-hour security, meeting rooms, reception areas, fitness centers, daycare facilities, and on-site food and beverages to attract tenants.
Subletting is usually prohibited, as landlords prefer to approve all tenants to maintain property continuity. Exceptions may be made for tenants facing financial hardship.
Leasehold Improvements
Tenants are allowed to perform construction and build out the space but may need to revert it to its original condition if required by the landlord.

CHAMPVA benefits
Are you the spouse or surviving spouse of—or a child of—a Veteran with disabilities or a Veteran who has died? If you don’t qualify for TRICARE (the Department of Defense’s health care program for active-duty and retired service members and their families), you may be able to get health insurance through the Civilian Health and Medical Program of the Department of Veterans Affairs (CHAMPVA). Through this program, we cover the cost of some of your health care services and supplies. This is called cost sharing. Find out if you qualify for CHAMPVA and how to apply.
Am I eligible for health care through CHAMPVA?
You may only be eligible for health care through CHAMPVA if you don’t qualify for TRICARE and at least one of these descriptions is true for you.
At least one of these must be true :
- You’re the spouse or child of a Veteran who’s been rated permanently and totally disabled for a service-connected disability by a VA regional office, or
- You’re the surviving spouse or child of a Veteran who died from a VA-rated service-connected disability, or
- You’re the surviving spouse or child of a Veteran who was at the time of death rated permanently and totally disabled from a service-connected disability, or
- You’re the surviving spouse or child of a service member who died in the line of duty, not due to misconduct (in most of these cases, family members qualify for TRICARE, not CHAMPVA).
A service-connected disability is a disability that we’ve concluded was caused—or made worse—by the Veteran’s active-duty service. A permanent disability is one that’s not expected to improve.
Note: A Veteran who’s the qualifying CHAMPVA sponsor for their family may also qualify for the VA health care program based on their own Veteran status. If 2 spouses are both Veterans who qualify as CHAMPVA sponsors for their family, they both may now qualify for CHAMPVA benefits. Each time they need medical care, they may choose to get care through the VA health care program or using their CHAMPVA coverage.
What else might affect whether I can get CHAMPVA benefits for myself or a family member?
There are other factors that may affect whether you or other family members qualify for CHAMPVA. Please click on the description that matches your status to learn more:
A new or expectant parent
If you’re expecting a baby, you’ll need to take the 2 steps listed here before you can apply for CHAMPVA for your newborn:
- You’ll need to get a Social Security number for your baby by applying at the nearest Social Security Administration office, and
- You’ll need to set up the baby’s status as a dependent of the Veteran sponsor by contacting your nearest VA regional office.
Medical claims can’t be paid until you sign your baby up under CHAMPVA, so please get them a Social Security number and set their status as a dependent as soon as possible.

A surviving spouse who’s currently remarried
If you’re the surviving spouse of a qualifying CHAMPVA sponsor and you remarry before age 55, you no longer qualify for CHAMPVA as of midnight on the date of your remarriage.
If you remarry on or after your 55th birthday, you can keep your CHAMPVA benefits.
A surviving spouse whose remarriage has ended by death, divorce, or annulment
If you’re the surviving spouse of a qualifying CHAMPVA sponsor and you remarry, but the remarriage ends by death, divorce, or annulment, you may qualify again for CHAMPVA.
The first date that you qualify again is the first day of the month after your remarriage ends or December 1, 1999—whichever date is later. You’ll need to provide us with copies of your marriage certificate and death, divorce, or annulment documents (as appropriate).
An 18-to-23-year-old dependent child of a Veteran
If you’re covered under CHAMPVA and you turn 18 years old, you’ll need to send us proof that you’re enrolled in college or another educational institution to keep getting benefits.

A Veteran’s stepchild who leaves the Veteran’s household
If you were covered under CHAMPVA as the stepchild of a Veteran, and you leave the Veteran’s household because of a divorce or remarriage, you no longer qualify for CHAMPVA.
A primary family caregiver of a Veteran with injuries and/or disabilities
If you’re a family member caring for a Veteran with disabilities, and you’re not entitled to care or services through another health plan, you may qualify for CHAMPVA.

If the Veteran you are caring for sustained or aggravated a serious injury (or illness) in the line of duty in the active military, naval or air service during any service era, you may qualify for health care benefits and other caregiver support through the Program of Comprehensive Assistance to Family Caregivers.

A CHAMPVA beneficiary who’s 65 years old or older—or who qualifies for Medicare at any age
CHAMPVA is always the second payer to Medicare. Here are some requirements you need to know:
If you’re under 65 years old, you’re eligible for CHAMPVA if you meet both of these requirements.
Both of these must be true:
- You have both Medicare Parts A and B, and
- You’re otherwise eligible for CHAMPVA
If you’re 65 years old or older, you’re eligible for CHAMPVA if you’re eligible for Medicare. If you turned 65 before June 5, 2001, and you’re entitled to either Medicare Part A or B, you’ll also need to enroll in Medicare Part B to be eligible for CHAMPVA.
Note: You don’t need to enroll in Medicare Part D to qualify for CHAMPVA.

What benefits do I get with CHAMPVA?
With CHAMPVA, you’ll be covered for services and supplies when we determine they are medically necessary and were received from an authorized provider. When providers are performing services within the scope of their license or certification, we consider them to be authorized.
Covered services include:
- Ambulance service
- Ambulatory surgery
- Durable medical equipment (DME)
- Family planning and maternity
- Inpatient services
- Mental health services
- Outpatient services
- Pharmacy (prescription medicines)
- Skilled nursing care
- Transplants

How do I get CHAMPVA benefits?
You’ll need to apply for these benefits.
To apply, submit these required documents:
Application for CHAMPVA Benefits (VA Form 10-10d) , and
Other Health Insurance Certification (VA Form 10-7959c) , and
Documents related to your Medicare status:
If you qualify for Medicare for any reason, you’ll need to submit a copy of your Medicare card.
If you’re 65 years old or older and don’t qualify for Medicare, you’ll need to send us documentation from the Social Security Administration that confirms you don’t qualify for Medicare benefits under anyone’s Social Security number.
To speed up the processing of your application, you can also send copies (not originals) of these optional documents:
- The page from the VBA rating decision showing your Veteran is permanently and totally disabled (or the death rating if you’re a survivor)
- Your Veteran’s DD214 (Certificate of Release or Discharge from Active Duty)—or, if the Veteran was a World War II or Korean War Veteran, the Report of Separation. If you don’t have a copy of the necessary form, you can request it by submitting a Standard Form 180, Request Pertaining to Military Records, from the National Archives. - A copy of each child’s birth certificate or adoption papers

If you’re a surviving spouse who remarried but is once again single, also send a copy of the legal document that ended your marriage. This may be a divorce decree, death certificate, or annulment decree.
Be sure to sign and date your application. If your Veteran is applying for you as a spouse, be sure to also include the date of the marriage on the application.

Once we get your application, we’ll review it to be sure it’s complete and includes all required forms. If it’s not complete, we’ll return it to you with more instructions.
How long will it take to hear back about my application?
If you send us all required and optional documents—and if your application is complete—it’ll take about 6 weeks after we get your package until you get your CHAMPVA ID card and related materials.
If you send us only the required documents, it may take 2 to 8 months since we’ll need to confirm your information with other federal agencies.

This form must be completed by each person, 18 years of age or older, applying for a Temporary Resident Visa, a study or work permit outside Canada.
Write the personal details about:
- If you are married and you were physically present at the marriage, choose “married - physically present” in the marital status box.
- If you are married and you were not physically present at the marriage, choose “married - not physically present” in the marital status box.
- If you are married and your spouse was physically present at the marriage, choose “married - physically present” in the marital status box.
- If you are married and your spouse was not physically present at the marriage, choose “married - not physically present” in the marital status box.
- your mother,
- your father.
Include: full name, relationship, date of birth, marital status (married, single, widowed, common-law, divorced, separated, annulled marriage), present address and occupation (job), and if they will come with you to Canada by checking “ Yes ” or “ No ”.
If a person is deceased, write “deceased” under “Present address”, and write the city and the date they died.
If not currently employed, please indicate if that person is retired, studying, etc.
You must answer all questions. If a section does not apply to you, write “Not applicable” or “N/A”.
Note: If you do not have a spouse or a common-law partner, read “ Note 1 ”, then sign and date the declaration at the end of Section A.
Section B - Children
Write the personal details about your children. It is very important that you list all of your children even if they are already permanent residents or citizens of Canada. This includes:
- married children;
- adopted children;
- children of your spouse (step-children) or common-law partner;
- any of your children who have been adopted by others; and
